Step 1: Create an EZ Facility Account

- Go to monumentice.ezfacility.com

- Click Login on the left side

- Select Create Account in the pop-up box

- Fill out your information and click Create Account

Step 2: Book Your Clinic

Log in to your EZ account

Go to Buy/Package:

Complete your registration and payment

**Each clinic purchase covers all listed sessions.

This is not a punch pass — missed sessions cannot be transferred or credited to another clinic.
